Playtest feedback form
A free playtest feedback form for video games, ready to copy: the questionnaire players fill in after a session and the sheet observers fill in during it.
A playtest gives you two kinds of evidence: what players tell you afterwards, and what you saw them do. This page has a form for each. Copy them as they are, or cut them down to the questions your round needs.
The player questionnaire
Hand this to each player straight after the session, before anyone talks about the game. Most players finish it in about ten minutes.
PLAYTEST FEEDBACK FORM Game: ____________ Build: ____________ Date: ____________ Player ID: ____________ ABOUT YOU 1. Which platforms do you play on? (PC / PlayStation / Xbox / Switch / Mobile / Other) 2. About how many hours a week do you play games? 3. Name two or three games you finished in the last year. FIRST IMPRESSIONS 4. How would you describe this game to a friend, in one sentence? 5. Which other game did it remind you of, and why? WHAT HAPPENED 6. What was the most frustrating moment? What were you trying to do? 7. Was there a moment you wanted to stop playing? When? 8. Was there anything you didn't understand? What was it? 9. What was the best moment of the session? HOW IT FELT (1 = strongly disagree, 5 = strongly agree) 10. I always knew what to do next. 1 2 3 4 5 11. The controls did what I expected. 1 2 3 4 5 12. The difficulty felt right for me. 1 2 3 4 5 13. I wanted to keep playing when we stopped. 1 2 3 4 5 LOOKING AHEAD 14. If you could change one thing, what would it be? 15. What each part is for
- About you checks that the player matches the people you sell to. Compare the answers with the spec you recruited against.
- First impressions shows whether your pitch survives a first session. If players describe a different game from the one on your store page, write that down as a finding.
- What happened points you to the moments worth finding in the recording. Ask for the moment first, then the reason.
- How it felt gives you four numbers to compare across rounds. Keep the wording identical every time so the comparison holds.
- Looking ahead ends on the player's own priorities, which often differ from the team's.
The observer sheet
Whoever watches the session fills this in live. Write down what the player did and what they said in separate columns. The two often disagree, and when they do, what the player did usually tells you more.

Did the session answer the round questions? ______________Running it online
To send the questionnaire as an online form, paste the questions into Google Forms, Microsoft Forms or Typeform. Set questions 10 to 13 as linear scales from 1 to 5 and the rest as short or long answers. Ask players to fill it in before they leave the call or the room, while the session is fresh.
Keep the questions open
A question that leads tends to get the answer it suggests. "Did you like the combat?" invites a yes, while "How did the combat feel?" lets the player tell you. The same goes for observers: note where a player got stuck and what they tried, and step in only if the session can't go on.
Our full approach to sessions is in how we run a focus group. If you'd rather hand the recruiting, moderation and report to someone else, see our playtests and focus groups.
